FAQ 04:

I've received a completed Medical Evaluation form indicating that the injured employee may return to work with no restrictions. Is there anything else I need to do?

• Be sure to forward a copy of the Medical Evaluation form to the Office of Risk Management and Insurance.
• In this case, no changes need to be made to the employee's payroll record.
• No additional follow-up treatment is required, unless specified on the Medical Evaluation form.

In all cases:

• An employee who suffers a work-related injury must report it immediately to his/her supervisor.
• The injured employee must provide their supervisor and/or payroll manager with a completed Medical Evaluation form prior to returning to work.
• The supervisor and/or payroll administrator must contact the Office of Risk Management and Insurance to advise them of the pending claim.
